Dr. Pouya Afshar, now CEO, and Dr. Ashkan Hayatdavoudi, now CMO, were seeing it on night shifts as hospitalists—patients bouncing in and out of hospitals without follow-up or support. Melody Shedlosky, RN MBA, now COO, was experiencing the same challenge from the ER command center. As a nurse executive and Lean Six Sigma Master Black Belt, she saw the system’s blind spots firsthand.
That shared frustration became a blueprint. That blueprint became Presidium Health.
The Problem We Saw
Pouya began his healthcare career with an MBA, then became a physician and matched into internal medicine at Scripps Clinic/Green Hospital in 2007. Ash, who was studying physiological systems modeling at UCLA, followed a similar path—attending medical school at UC Irvine and joining the same residency in 2009.
During night shifts, they saw the same patients caught in repeat cycles of discharge and readmission. These weren’t isolated cases—they were signals of a broken system.
The Seed of a New Model
In 2010, Pouya became a nighttime hospitalist at UCSD and expanded into SNFs. Ash joined in 2012. By 2015, Pouya began seeing patients at home, growing a mobile practice that would later become Integrated Healthcare Alliance.
While in law school at the University of San Diego, Ash remained in close contact. In 2017, over a game of bocce ball, they envisioned a company for patients no one else knew how to serve.
From Idea to Infrastructure
That vision became Presidium Health. In 2019, Melody Shedlosky joined as COO, completing a triad of clinical, strategic, and operational leadership. With a foundation rooted in real-world care, the model was ready to scale.

Dr. Pouya Afshar, MD MBA – Chief Executive Officer
A physician and health systems strategist with deep operational knowledge and a rare combination of business and clinical acumen. Pouya leads Presidium’s long-term vision, external partnerships, and contract development. He brings a values-driven, patient-first lens to every decision.
Dr. Ashkan Hayatdavoudi, MD ESQ – Chief Medical Officer
With dual training in medicine and law, Ash drives clinical innovation, risk strategy, and regulatory design. He oversees model fidelity and care quality across regions, and leads our in-house legal structuring for value-based arrangements. He is also the architect of our compliance-informed tech stack.
Melody Shedlosky, RN MBA – Chief Operating Officer
A Lean Six Sigma Master Black Belt, Melody previously managed San Diego’s largest ER. At Presidium, she leads workforce design, operational scaling, and quality execution. Her insight into clinical flow and frontline dynamics drives our high-reliability care delivery across sites.
Laurent Bortolamiol – VP of Technology
Laurent brings a clinician-informed approach to health tech, focusing on usability, interoperability, and systems that work in the real world. He leads all tech infrastructure and integrations, reporting to Dr. Hayatdavoudi.
Kenneth Huang – VP of Finance
Kenneth is a finance leader with a background in value-based contracts, payer relationships, and analytics. He ensures that fiscal strategy is aligned with our care model and oversees all revenue, reconciliation, and forecasting—reporting to Melody Shedlosky.
