Care Coordination
All patients in this program receive a primary point of contact called a Lead Care Manager. The Lead Care Manager coordinates all of your care needs and services. Individuals are active participants in their care plan development and implementation, ensuring their voices are heard and their preferences are respected.
Resource Engagement
Information about available resources is challenging to understand and difficult to find. We are experts in resources management. We will work with you to identify what resources would be of most benefit to you, what resources you are eligible for, and stay by your side every step of the way until you are actively engaged in the respective resources.
Transitional Care
It can be overwhelming to move through a transition of care. Whether it be going from home to hospital, home to emergency department, hospital to skilled nursing facility (or any other environment), we will be there to support you. Utilizing our hospital at home care model, we ensure continuity of services, timely visits, transportation, and medication management.
